• If you are purchasing a membership as a gift, please fill out the order form with the membership recipient’s information (name, address, spouse, number of children, phone) and your credit card number
   
• Please fill in your information (name, address, phone) under “Billing Details”
   
• If you would like the membership sent directly to your recipient, please enter their information under “Shipping Details”
